What Youll Do- Provide skilled nursing care and education to patients at home
- Document and evaluate patient responses to interventions
- Collaborate with healthcare teams and community partners to coordinate care
- Monitor health status and promote wellness, prevention, and rehabilitation
- Supervise and guide other nursing staff as needed
- Exercise independent judgment within established protocols
- Maintain detailed and accurate patient records, ensuring confidentiality
- Adapt to flexible schedules and occasional on-call duties
- Drive to patient homes, including in inclement weather conditions
What You Need- Active MA Registered Nurse license (must upload license with application)
- Minimum 2 years of relevant nursing experience
- Ability to work independently with strong critical thinking skills
- Excellent interpersonal communication skills
- Valid drivers license and ability to drive safely
- Flexibility regarding work hours and on-call responsibilities
