What Youll Do
- Manage the healthcare needs and coordinate services for patients at Berkshire Medical Center
- Review patient admissions and ongoing care needs to determine appropriate levels of care
- Facilitate transitions to post-hospital care, ensuring smooth patient handoffs
- Collaborate with multidisciplinary teams to optimize patient outcomes
- Monitor patient progress during hospitalization and perform quality assurance activities
- Participate in performance improvement initiatives within Berkshire Health Systems
- Maintain accurate documentation and review data for reports and audits
What You Need
Required:- Valid Massachusetts Temporary Nursing Service Agency License
- Proven experience in case management or hospital nursing
- Strong organizational, communication, and interpersonal skills
- Knowledge of Medicare, Medicaid, and third-party payer systems
- Computer literacy and data management abilities
Preferred:- Experience with healthcare resource coordination and community resources
- Ability to work independently and manage multiple priorities
- Knowledge of local healthcare resources and reimbursement processes
