Description

Requirements : 

  • 1+ year of PT experience – Required

  • LTC or Rehab experience – Required

  • Active Massachusetts PT license – Required

  • New graduates are welcome with Level II clinical experience; Level I observation-only experience does not qualify.

    Responsibilities : 

  • Evaluate and rehabilitate patients with physical functioning disorders and/or wound care needs.
  • Develop and implement appropriate plans of care following regulatory and clinical practice standards.
  • Utilize standardized assessments and evidence-based practices.
  • Provide manual techniques, ambulation training, therapeutic exercises, modalities, supportive/assistive devices, wound care, and other appropriate interventions.
  • Monitor patient progress and modify treatments based on clinical outcomes.
  • Evaluate treatment effectiveness and make referrals when appropriate.
  • Provide education, consultation, and counseling to patients, families, caregivers, and other healthcare providers.
  • Communicate effectively with the healthcare team regarding patient progress, barriers, and treatment plans.
  • Complete comprehensive discharge summaries and maintain accurate electronic documentation.
  • Accurately record treatment minutes in accordance with regulatory requirements.
  • Participate in patient care conferences, utilization review meetings, rehabilitation meetings, trainings, in-services, and other required activities.
  • Obtain necessary DME and supplies as needed.
  • Supervise and support Physical Therapist Assistants, rehabilitation aides, and/or students.
  • Practice in accordance with APTA standards and Massachusetts state regulations, ethics, and professional requirements.
