What You'll Do
- Conduct patient screenings and evaluations for cognitive and ADL physical functioning disorders
- Develop and implement individualized care plans following regulatory standards
- Administer rehabilitative and skilled treatments, including ADL retraining, therapeutic exercises, and use of assistive devices
- Monitor patient progress, modify treatments as needed, and document outcomes
- Provide consultation to patients, families, and care teams regarding treatment plans
- Maintain accurate and compliant documentation in electronic health records
- Participate in patient care conferences, meetings, and trainings
- Supervise and support OT assistants, rehab aides, and students as applicable
What You Need
- Valid occupational therapy license in Oklahoma
- NBCOT certification
- At least 1 year of relevant experience in LTC or skilled nursing facilities
- Strong clinical skills in cognitive and ADL therapy
- Excellent communication and team collaboration skills
- Ability to adhere to professional standards and practice acts
