What Youll Do
- Screen and evaluate patients with cognitive and/or ADL physical functioning disorders
- Develop and implement individualized care plans following regulatory standards
- Provide rehabilitative, skilled, and medically necessary treatment interventions such as ADL training, therapeutic activities, exercises, modalities, and use of supportive devices
- Assess and document patient progress; modify treatment plans accordingly
- Evaluate treatment effectiveness and adjust services based on outcomes
- Communicate with patients, families, caregivers, and healthcare team members
- Prepare detailed discharge summaries and maintain accurate documentation
- Participate in care conferences, team meetings, and ongoing training sessions
- Support and supervise occupational therapist assistants and rehabilitation aides
What You Need
- Current Missouri Occupational Therapist license
- BLS/CPR certification
- At least 1 year of OT experience in a long-term care environment
- Strong communication, organizational, and clinical skills
- Ability to work effectively within a multidisciplinary team
