What Youll Do:- Provide skilled nursing care to patients in their homes based on Physicians orders and care plans
- Document all nursing care and patient responses accurately and promptly
- Collaborate with healthcare teams and family members to coordinate care
- Promote health, prevent illness, and assist with rehabilitation efforts
- Exercise independent judgment and critical thinking to address patient needs
- Follow protocols, policies, and procedures specific to each care unit
- Be flexible with work hours and willing to accept on-call responsibilities
What You Need:- Valid Massachusetts LVN or LPN license
- Ability to travel within the service area and drive in various weather conditions
- Strong interpersonal and communication skills
- Ability to maintain confidentiality and records accurately
- Commitment to dependability and adherence to work schedules
- Willingness to work flexible hours, including on-call duties
- No more than 5 days of requested time off during the assignment; must be noted upfront
