    Job Summary: In addition to meeting the basic standards of acute Case Management, the Team Leader is expected to role model and exceed staff Standards of Performance. S/he is to assume accountability for the quality of case management services delivered to our patient population. The Team Leader provides direct case management services, as needed, and oversees the services provided by other case managers and social workers in the department. The Team Leader assists management with the coordination and orientation of new staff members, contributes to the evaluation of new and seasoned staff performance, and provides continuing education. In conjunction with the Manager, the Team Leader is also responsible for planning and managing the resources necessary for the patients we serve as well as the department. The Case Management Team Lead must be able to demonstrate knowledge of growth and development to coordinate patient care appropriate to the age of patients served on his/her assigned clinical area. Employee must be able to successfully complete job specific orientation, Case Management specific competencies, and meet/exceed standards as identified in the criteria-based performance appraisal.

     

    Minimum Education, Training, and Experience:

    Required Qualifications:

      1. Masters or Doctoral Degree in Social Work
      2. Active social work license appropriate to degree level.
      3. Must show proficiency in completing assessments, treatment planning, aftercare planning and crisis management.
      4. Communication skills must be exceptional with the ability to interact professionally with professionals of multiple disciplines.
      5. Minimum of 5 years Case Management experience required.
