Position Summary

The Team Care Coordinator plays a vital role in supporting our physicians and clinical team by coordinating patient care, managing provider schedules, and ensuring efficient clinic operations. This position works closely with providers to document patient visits, coordinate surgeries and therapy orders, and deliver exceptional service throughout the patient experience.

Key Responsibilities

  • Accurately document patient visits by accompanying providers during examinations and recording medical histories, physical exams, procedures, and treatment plans
  • Handle and triage patient phone calls in a professional and timely manner
  • Schedule office visits, procedures, and surgeries
  • Manage physician and physician assistant clinical and surgical schedules
  • Submit and renew physical therapy orders
  • Call in prescriptions and medication refills as authorized by the provider
  • Coordinate patient care while maintaining accurate documentation and communication
  • Maintain patient confidentiality and comply with HIPAA guidelines
  • Support providers and clinical staff to ensure efficient daily operations

Qualifications

  • High school diploma or GED preferred
  • Medical background preferred, including Anatomy coursework, Certified Medical Assistant (CMA), or healthcare-related degree
  • Surgery scheduling experience required
  • Athena experience preferred
  • Knowledge of orthopedic healthcare preferred
  • Strong organizational, communication, and interpersonal skills
  • Excellent typing and computer proficiency (40 WPM preferred)
  • Ability to multitask, prioritize, and work effectively in a fast-paced environment
  • Strong attention to detail and ability to maintain accurate medical documentation
  • Ability to work independently while providing outstanding patient service

Work Environment & Physical Requirements

  • Standard medical office environment
  • Frequent sitting with occasional standing and walking throughout the day
  • Ability to use computers and standard office equipment
  • Travel between clinic locations may be required
