S.P.A.R.C. (Sobriety, Peace, Awareness, Recovery, Center) is seeking a qualified Targeted Case Manager to provide professional case management services to individuals in need of behavioral health support.
Responsibilities:
Conduct assessments and develop individualized service plans.
Provide ongoing case management, advocacy, and linkage to community resources.
Coordinate care with providers, agencies, and natural supports.
Monitor client progress and adjust service plans as needed.
Maintain accurate documentation in compliance with state and agency standards.
Work in person from Russell Springs. Hours are 8 am to 4 pm, Monday through Friday.
Qualifications:
Bachelor’s degree in human services (social work, psychology, counseling, or related field).
A year of work experience after receiving college degree.
Strong organizational, communication, and interpersonal skills.
Ability to work both independently and as part of a team.
