At Pathways to Life, Inc., we believe behavioral healthcare extends beyond therapy appointments and medication management. Many individuals facing serious mental health and substance use challenges also struggle with housing instability, transportation barriers, medical needs, unemployment, food insecurity, and fragmented systems of care.

We are seeking compassionate, organized, and motivated professionals to join our team as Tailored Care Managers (TCMs) serving the Greenville area. This role is ideal for individuals passionate about advocacy, behavioral health, crisis prevention, and helping people build long-term stability within their community's 

Overview

A Tailored Care Manager serves as the primary coordinator and advocate for Medicaid members with serious mental health disorders and substance use disorders. The TCM works to ensure behavioral health treatment, medical care, housing supports, transportation, benefits, and community resources function together as one integrated care plan.

This position provides whole-person, community-based care coordination for individuals with complex behavioral health needs and high-acuity service involvement.

Key Responsibilities
Behavioral Health Care Coordination
  • Develop and maintain person-centered care plans
  • Coordinate behavioral health services including outpatient therapy, psychiatric care, substance use treatment, MAT services, Peer Support, ACTT, CST, and residential services
  • Monitor member engagement and treatment participation
  • Identify barriers to treatment and service gaps
  • Support transitions following psychiatric hospitalization, detoxification, or residential treatment
Member Advocacy & Support
  • Assist members with accessing Medicaid-covered services
  • Help individuals navigate healthcare systems and community resources
  • Support applications for benefits, housing resources, transportation, and social services
  • Advocate for medically necessary services and continuity of care
Crisis Prevention & Intervention
  • Conduct outreach for members at risk of disengagement or crisis
  • Coordinate crisis response services and follow-up care
  • Monitor high-risk members to reduce emergency department visits and inpatient admissions
  • Participate in discharge planning and post-hospitalization follow-up
Care Team Collaboration
  • Coordinate communication among providers, family members, hospitals, therapists, psychiatrists, social services agencies, and community supports
  • Participate in treatment team meetings, staffing meetings, and interdisciplinary coordination efforts
  • Maintain collaborative relationships with internal and external providers
Documentation & Compliance
  • Complete Medicaid-required documentation and assessments
  • Maintain accurate progress notes and care management records
  • Track quality measures and care plan updates
  • Ensure compliance with NC Medicaid Tailored Care Management requirements

Minimum Education & Experience Requirements

Applicants must meet ONE of the following qualification pathways:

  • Bachelor’s degree in a Human Services field from an accredited college or university and a minimum of two (2) years of experience working directly with individuals experiencing mental health disorders, substance use disorders, intellectual/developmental disabilities, traumatic brain injuries, or related high-need populations

OR

  • Bachelor’s degree in a non-Human Services field from an accredited college or university and a minimum of four (4) years of experience working directly with individuals experiencing mental health disorders, substance use disorders, intellectual/developmental disabilities, traumatic brain injuries, or related high-need populations
