    Overview

    The Supervisor of Care Coordination provides effective leadership support by the delegation, coordination and integration of services within their assigned departments to assure the provision of quality care. Serves as a resource for the Care Coordination Team and is responsible for maintaining productive workflows toward the goals of the program. This position reports directly to the Administrator of Medical Surgical and Care Coordination Services and provides supervision and direct support to their teams.

    Responsibilities

    Essential Responsibilities:

    • In coordination with leadership, responsible for coordinating and directing patient care. Applies both clinical and

    operational management skills to meet the established quality, patient satisfaction, financial, and operational goals of the department.

    • Supports leadership in ensuring compliance with Joint Commission, federal, state and other applicable regulatory

    requirements. Implements program philosophy, objectives and standards of care on consultation with leadership.

    • Leads the Interdisciplinary Care Coordination Team of patient care providers along the continuum of care to reconcile the Plan of Care and manage hand-offs, including the Pharmacist, Social Worker, Primary Care Physicians, Specialists, Home Healthcare Agencies, Skilled Nursing Facilities and the acute care setting.
    • Observes and coaches staff in performance of duties and interpersonal interactions. Evaluates educational needs of staff. Participates in the orientation and/or on the job training of personnel.
    • Collaborates with department leadership in the hiring and termination of staff, progressive discipline and staff

    evaluations.

    Qualifications

    Education Required: Graduate Accredited School of Nursing

    Preferences: BSN. In lieu of BSN, commitment to earn BSN within agreed upon time frame may be required.

    Experience Requirements: 3 to 5 years Clinical Acute Care

    Licensing Requirement: Active FL Nursing License, Basic Life Support (BLS)

    BLS provided by the American Heart Association (AHA), required within 30 days of hire.
