Supportive Housing Worker

Location: Cape Girardeau, MO

Pay: $16.42 - $18.82/hour (depending on experience)

Work Schedules Available:

• Full - Time 11:30am - 8:00pm Wednesday/Thursday off
• Part - Time: 5:00pm - 8:00pm Tuesday - Sunday (off on Monday''s)

Qualifications:

• High School Diploma/HSE/or GED
• Must hold Valid Driver''s License (Class E for Missouri or Class D for IL Residents - Obtain on start date) and current continuous auto insurance.

About the Program:

Clustered Apartments allow for on-site monitoring and on-site interventions of individuals who require an intensive level of care and support, and who may require skills in the necessities of daily living. Clustered Apartments provide low income subsidized housing assistance to persons with disabilities, mental illnesses, and/or poverty.

Job Summary:

Responsible for coordination between approved referrals for the program and landlords during the application/move-in process.

Job Duties:

• Communicate with the Clustered Apartment Supervisor and Team Leader regarding potential referrals and assist with the approval process.
• Communicate with partnering landlords to identify vacancies and potential applicants based on approved referrals.
• Assist potential applicants with completion of all required paperwork by the landlord and be the point of contact for landlord regarding applicants.
• Serve as resource for clients throughout the application/move-in process until the client has successfully moved into the approved clustered apartment location.
• Coordinate with the client''s case manager during the application/move-in process to ensure the case manager is attending to the client''s needs to finalize move-in (SCL funding requests, furniture, utilities, etc.)
• Attend agency staffing to discuss upcoming vacancies and to assist with program recruitment.
• Document according to clinical policies and procedures, DMH, CARF, and any other federal or state program per agency standards.
