JOB DESCRIPTION POSITION TITLE: Supportive Housing Program ManagerEXEMPTION STATUS: ExemptREPORTS TO: Director of Scattered-Site Housing ProgramsPOSITION DESCRIPTION: Plans, implements, directs, and monitors all aspects and services related to homeless outreach and permanent supportive housing programs using a Housing First framework for a diverse set of formerly homeless individuals and families who are impacted by chronic medical issues Chicago House empowers persons living with or vulnerable to HIV/AIDS to lead healthy and dignified lives, through housing and compassionate, client-centered support services.
