ABOUT THE POSITION

The Support Service Worker II reports to a Public Health Unit Supervisor and is part of the Healthy Families Marin Home Visiting Unit within the Division of Public Health in the Health and Human Services Department (HHS) at the County of Marin.

Healthy Families Marin partners with prenatal individuals, new parents, families, and children from birth to age five to strengthen the parent-child relationship and promote healthy child development. This position is assigned to provide home visiting services and requires regular in-person, on-site work, including conducting visits in families' homes.

Under general supervision, the Support Service Worker II (Home Visitor Assignment) performs duties to support families through relationship-based home visiting using the Healthy Families America (HFA) Best Practice Standards. The home visitor builds and sustains trusting relationships with families, engages them in goal setting, and provides education, information, referrals, and service coordination to support family well-being and child development.

The Support Service Worker II conducts strength-based home visits informed by parent-child observations and standardized developmental screenings, assists families in understanding typical child growth and development, and supports parents in identifying goals and next steps. The position coordinates services with community partners and public benefit programs, acts as a liaison between families and service providers, and supports families in accessing preventive health care and community resources.

This role is also responsible for maintaining accurate, detailed, and timely documentation in accordance with program and County requirements, including case notes, assessments, and reports. The Support Service Worker II works collaboratively with a multidisciplinary team, participates in reflective supervision, and contributes to outreach and recruitment efforts to engage families in services.

There are currently two (2) regular, full-time Support Service Worker II vacancies in the Healthy Families Marin Home Visiting Unit.

ABOUT YOU

OUR HIGHLY QUALIFIED CANDIDATE

Our highly qualified candidate has at least one (1) year of evidence-based home visiting experience and/or early childhood education experience (ages 0-5 years old) in one of the following roles: teacher, educator, family childcare provider, etc. They have a strong foundation in early childhood development, family-centered practice, and relationship-based service delivery. They are skilled at building trust with families from diverse cultural, economic, and social backgrounds and approach their work with warmth, respect, and cultural humility.

The highly qualified candidate is organized and detail-oriented, with the ability to maintain accurate, timely documentation in computerized databases and consistently arrange and sustain home visiting appointments. They are comfortable conducting home visits, gathering and documenting observations, administering standardized screening tools, utilizing age-appropriate activities, child development centered approaches, and program guidance and supervision to inform visit planning and follow-up.

They communicate clearly and effectively, both verbally and in writing, and are able to collaborate with families, colleagues, and community partners to coordinate services and connect families to appropriate resources, including Medi-Cal, WIC, CalFresh, and other community-based supports. Experience navigating county systems and community resources is highly desirable.

The highly qualified candidate values reflective practice and professional growth, responds well to constructive feedback, and actively engages in reflective supervision to strengthen skills, maintain professional boundaries, and support high-quality services. QUALIFICATIONS

Knowledge of:

  • The ethnic, economic, and social factors affecting residents and special populations of the community or clients served by Health and Human Services programs.
  • Major community resources including social, employment and health services.
  • Ethnic, economic and social barriers which inhibit residents from seeking and receiving services and methods of overcoming or lowering service providing barriers.
  • Acceptable household management and personal care standards such as basic nutrition, economical meal planning, money management and budgeting and personal health and hygiene standards, methods and practices.
  • Normal health needs of infants, children, adolescents, adults, and aged persons.
  • Signs and symptoms requiring medical assistance.
  • Interviewing and information gathering techniques.
  • Computer applications related to the work.
  • Standard office practices and procedures, including filing and the operation of standard office equipment.

Ability to:

  • Evaluate client needs through interviewing and observation.
  • Perform a variety of paraprofessional support work.
  • Resolve conflicts between clients and providers.
  • Recognize specific indications of need for various services and developing service delivery plans.
  • Maintain accurate records and files.
  • Make accurate complex arithmetic calculations.
  • Prepare reports related to the work performed.
  • Establish and maintain effective working relationships with clients, children, representatives of the community served, employees of County departments, other agencies and the public.

MINIMUM QUALIFICATIONS

Three years of experience in a health, social service or community organization that demonstrates possession of the stated knowledge and abilities plus successful completion of twelve (12) semester units of education in health science, behavioral science and/or social welfare from a recognized college or equivalent. One additional year of experience may substitute for the educational requirement. College-level education in a field as listed above may be substituted for the experience on a year-for-year basis to a maximum of two years.
