Description
Under the direction and guidance of the Chief Executive Officer, The Supervisor of Community Case Management is responsible for overseeing the administration, compliance, and operations of participating in Continuum of Care (COC) agencies under Housing and Urban Development's (HUD) guidelines and advancing Valley Healthcare's mission to end homelessness in the Chattahoochee Valley. This position is responsible for the development, implementation and evaluation of systems and processes related to partnerships in the community and case management. The Supervisor of Community Case Management is also responsible for ensuring that the department provides high-quality technical assistance along with effective resources within the community that are responsive to the needs of the clients, families, and other members in the community.

Education and Experience:
Bachelor of Social Work (BSW) degree preferred or a minimum bachelor's degree in human service field and two years' experience in the following:
Evaluation: The evaluation of work performance, including: specific duties, responsibilities, demeanor, and work ethic, will be ongoing and implemented by the Supervisor per center policies.

Security (HIPAA): Every employee is required to protect their computer log-on and password. You must secure your work area prior to leaving it unattended for an extended period of time. You are required to log-off of your computer and secure your work area prior to leaving at the end of the day. If you have a responsibility for closing or locking files, securing equipment, and you do not comply with these requirements, you will be subject to reprimand and/or termination based upon the infraction. We are required by law to protect patient privacy, identifiable patient information, and all properties containing such information.
Physical Requirements: Prolonged periods of sitting at a desk and working on a computer. Must be able to lift up to 15 pounds at times.

Founded in 1969 by West Virginia University, Valley Counseling Services united with The Human Resources Association and incorporated as Valley Community Mental Health Services in 1972. Doing business as Valley HealthCare System, a nonprofit corporation, Valley has grown from a small counseling center located in the basement of a University building to a large multi-county, comprehensive behavioral health program.
Spurred by the federal government, funds became available through the passage of the Community Mental Health Center Act of 1963, introduced by President John F. Kennedy and Valley was able to establish itself as one of 13 federally-funded mental health centers in the state in the early 1970’s.
Over the years, Valley has continued to add to its array of services for people with disabilities, including services for individuals with developmental disabilities, mental health and chemical dependency issues.
We believe that the work that we do in Monongalia, Marion, Preston and Taylor Counties enhances the quality of life, not just for the persons served, but for everyone in those counties as well.
