APPLICATION REQUIREMENTS FOR SUBSTITUTES Once all these are items are completed and have the proper documentation,please call to set up an appointment withAlison Werkmeister, 973.729.2155 Ext. 1402 The following documents must be submitted for approval by the Board of Education: Completed Sparta Application thru Applitrack via www.Sparta.org Substitute Information Sheet Drivers License CDL (Bus Drivers) SS Card Federal W-4 NJ W-4 I-9 PTT - NJ Sexual Misconduct and Abuse Disclosure Release (dating back 20 years for all employment directly involvingchildren) Direct Deposit w/Voided check Mantoux Test - copy of test within the last 6 months or one can be completed here in the district Criminal History Clearance - (You can look online (https://homeroom5.doe.state.nj.us/chrs16/?app-emp-history) for thisabout 2 weeks after application or archive, Transfers are available a few days after they are submitted ) New Applicant Archive (Applicants previously fingerprinted for the Department of Education and Approved Subsequent to February2003) Transfer (Only Substitutes & Bus Drivers are Eligible) NJ Teaching Certificate (Teachers Only) OR New Jersey Substitute Teacher Certificate (Teachers & Nurses Only) To apply for a substitute credential (initial or renewal), educators must create an account in NJEdCert.https://www.nj.gov/education/certification/substitutes/index.shtml . Educators should follow thedirections provided on each screen to complete the application. Once they submit the fee, they willbe able to upload their fingerprint clearance directly to their account, and the college must submittheir official transcript via email to certapplication@doe.nj.gov . For renewals, if the educator hasbeen continuously employed, with no break-in service, the district/school/vendor may provide theeducator with verification of continuous employment to include with the application.
