Rate of Pay: $27.00 to $30.00 Per Hour Depending on Experience
MHM Services, a Centurion company, is proud to be the provider of mental health services to the California Department of State Hospitals-Forensic Assertive Community Treatment Program (FACT). The FACT program is a statewide system that provides an unlocked, supervised living environment with 24-hour staffing. FACT's model of care is centralized services with a slow transition to community-based services. Services needed to support community reintegration include but are not limited to the following:
- Forensic mental health treatment through individual and group therapy settings
- Life skills training
- Day socialization groups
- Collateral contacts
- Home visits
- Substance abuse screenings
- Psychiatric services
- Case Management
- Court reports
- Psychological assessments
Schedule: Monday through Friday; 8:00am to 4:30pm
Under the direction of the Program Director the Substance Abuse Counselor is responsible for providing support, guidance, and mentoring the patients in a community setting to meet service delivery requirements of the FACT Program.
Essential Duties/Responsibilities
- Provide intake, assessment, treatment planning, group and individual counseling to clients in the Substance Use Disorders/Addiction program
- Provide after-care plan for clients returning to the community including program referrals to substance use disorders/addiction treatment
- Maintain accurate and thorough clinical records per Company and DSH requirements in a timely manner
- Provide input and reports to the Clinical Supervisor regarding the clinical needs of clients and programming, improving ways of offering prevention, outreach, and treatment services
- Work collaboratively with behavioral/mental health and medical staff when clients have co-morbid conditions
- Communicate with treatment and case management staff through daily briefing and debriefing between shifts
- Attend required staff meetings, safety training and in-service programs as required
- Identify and maintain professional boundaries to preserve the integrity of treatment and the professional relationship
- Proactive in performance improvement activities as demonstrated by participating in the Continuous Quality

Qualifications
- Certified Alcohol and Drug Counselor (CADC) certification preferred, but will accept other Alcohol and Drug Counseling Certifications
- Must be able to obtain CPR certification
- Must be able to pass background investigation and obtain agency security clearance where applicable
- Valid State Driver's License
