Special Education Transition Specialist LCHS
Position Summary
The Transition Specialist is responsible for supporting high school students with a disability or IEP in successfully completing their education and preparing for postsecondary opportunities. This role focuses on reducing the dropout rate, increasing graduation rates, and ensuring students have access to academic, career, and transition support services. The Transition Specialist works collaboratively with teachers, counselors, administrators, parents, and community partners to monitor student progress and implement strategies that promote student success.
Key Responsibilities
o Develop and implement strategies to reduce dropout rates and increase graduation rates.
o Monitor student academic progress, attendance, and behavior to identify at-risk students early.
o Facilitate credit recovery opportunities and support students in regaining academic standing.
o Partner with teachers, counselors, administrators, and support staff to provide coordinated services for students.
o Maintain consistent communication with students and parents/guardians regarding progress toward graduation.
o Participate in IEP/transition planning meetings and contribute to individualized student transition goals.
o Organize and coordinate annual job fairs, career exploration events, and community networking opportunities.
o Provide students with resources on postsecondary education, training programs, employment, and community services.
o Support students in developing self-advocacy, employment, and independent living skills.
o Maintain communication logs, progress notes, and detailed records of student interactions.
o Track and evaluate the effectiveness of interventions, credit recovery, and graduation support services.
o Use data to measure program success and adjust strategies as needed.
Qualifications
Work Environment
Work 4 hours per week at $30 per hour not to exceed $5,000.00 per school year.
