    Stroke Program Nurse Coordinator, Full Time, 8am-4:30pm, Chestnut Hill Hospital - (263142)

    Description

     Facilitates comprehensive care for stroke patients by coordinating the continuity of care from admission to discharge through collaboration with interdisciplinary team members. The Stroke Program Coordinator facilitates the implementation of the Stroke Care Guidelines and other evidence/research-based practice and ensures that Hospital's Stroke Program incorporates the recommendations for Primary Stroke Centers published by the Brain Attack Coalition and the American Stroke Association.

    Education
    Bachelor's Degree Bachelor of Science in Nursing Required
    Master's Degree Master of Science Degree in Nursing Preferred

    Experience
    3 years experience in adult health with a focus on care of the stroke patient throughout
    the stroke continuum required. Required
    General Experience experience in adult health with a focus on care of the stroke patient throughout
    the stroke continuum required. Required
    General Experience in a clinical leadership role Preferred

    Licenses
    PA Registered Nurse License Required
    Basic Life Support Required
    Multi State Compact RN License Required Or
