CoreMedical Group Perm is seeking a Registered Nurse (RN) Hospice for a nursing job in Framingham, Massachusetts.

Will look at RNs with LTC experience! Hospice/Home Health experience preferred.

40-hour salaried full-time positions with participation in on-call coverage as required.
$85,000-$97,000 annually.

Census throughout the state of Mass is close to 100.

Key responsibilities include:
- Completing comprehensive patient and family assessments
- Developing and revising individualized plans of care
- Providing skilled nursing care and symptom management
- Coordinating care with physicians, facilities, families, and the interdisciplinary team
- Managing documentation timely and accurately within the EMR system
- Ordering medications, DME, and supplies as appropriate
- Supervising ancillary staff and coordinating visit schedules
- Participating in on-call rotation and IDG meetings


The ideal candidate will have:
- Current RN licensure
- Hospice and/or home health experience preferred
- Strong assessment and critical thinking skills
- Excellent organizational and communication abilities
- Ability to work independently while collaborating with the team
