Registered Nurse (RN) - Case Manager

Location: Worcester County, MA
Job Type: Full-Time | Permanent | Direct Hire
Schedule: Days | 40-hour salaried position | 8:00 AM - 5:00 PM

Compensation
$85,000 - $97,000 annually

Join an established hospice organization with approximately 20 years of experience and a strong, long-tenured team. Hospice/Home Health experience is preferred, but experienced LTC RNs will also be considered. This role focuses on compassionate, patient-centered end-of-life care using a whole-person mind, body, and spirit approach.

Responsibilities:

• Complete comprehensive patient and family assessments
• Develop and revise individualized plans of care
• Provide skilled nursing care and symptom management
• Coordinate care with physicians, facilities, families, and the interdisciplinary team
• Maintain timely and accurate EMR documentation
• Order medications, DME, and supplies
• Supervise ancillary staff and coordinate visits
• Participate in on-call rotation and IDG meetings

Requirements:

• Current RN license
• Hospice/Home Health experience preferred
• LTC experience considered
• Strong assessment and critical thinking skills
• Excellent communication and organizational abilities
• Ability to work independently and collaboratively
• Compassionate approach to end-of-life care
