RN Case Manager – Breast & Cervical Care | Full-Time
Juneau,Alaska
️ Permanent Full-Time | Day Shift
Position Highlights:
Provide comprehensive care management for Breast & Cervical cancer prevention, screening, and diagnostic services
Perform comprehensive patient assessments and develop individualized care plans
Coordinate referrals, follow-up care, and continuity of services
Administer medications and treatments safely
Provide patient and family education
Advocate for patients and assist with navigating the healthcare system
Collaborate with providers, radiology, Community Wellness, and interdisciplinary teams
Maintain timely and accurate electronic health records
Participate in quality improvement, patient safety, and professional development
Provide guidance and delegation to ancillary staff
Float to other departments or assignments as needed
Requirements:
3 Year experiance RN Case Manager
Associate Degree in Nursing
Current, unrestricted Alaska RN license
AHA BLS required
Case Management Certification required within 2 years of hire
Clinical Competency Assessment required
High-Risk Competency required
1+ year of experience
Strong clinical judgment, organization, communication, and care coordination skills
Ability to work independently and collaboratively in a fast-paced environment
