Position Overview

A respected nonprofit home health organization is seeking an experienced Patient Care Manager (PCM) to oversee clinical operations, patient care coordination, and an interdisciplinary field team.

Reporting directly to the Director of Nursing, this is a hands-on leadership opportunity for a Home Health RN who enjoys mentoring clinicians, improving patient outcomes, and ensuring exceptional care in patients' homes.

Responsibilities

• Oversee daily home health clinical operations

• Lead, coach, and support interdisciplinary field staff

• Monitor patient care plans and episode management

• Ensure quality, compliance, and clinical excellence

• Coordinate staffing, resources, and patient needs

• Address clinical and operational challenges

• Promote staff accountability and professional development

Requirements

• Registered Nurse (RN) with strong Home Health experience

• Home Health clinical leadership experience preferred

• Strong understanding of Plans of Care and episode management

• Excellent clinical judgment and organizational skills

• Ability to mentor staff and manage competing priorities

• Collaborative, approachable leadership style
