    Seeking Speech-Language Pathologists in the Home Health setting for Fort Worth and the surrounding areas!

    Per Visit Rates:

    $75 per regular ST Visit

    $112.50 per ST Eval

    Job Description:

    The Speech-Language Pathologist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase.

    • Evaluate patients and perform appropriate therapy interventions

    • Develop a comprehensive plan of care and coordinate multi-disciplinary care

    • Perform clinical documentation and visit notes

    • Continued communication with physician, care team members, patient and family

    • Comfortable working in all aspects of the home health setting

    • Implement and maintain SLP protocols

    • Manage patient treatment and outcomes directly

    Requirements:

    • Current Texas Speech-Language Pathology License

    • One year of SLP experience

    • Experience with Homecare Homebase is helpful, but not required

    • Previous Home Health experience is helpful, but not required

    • Current Texas driver's license

    • CPR certification
