Medical Social Worker (MSW) PRN

Make a Difference Beyond the Clinical Visit

Elara Caring is hiring a compassionate PRN Medical Social Worker to support home health patients and families throughout Portland and surrounding southern Maine communities.

This position gives you the independence to work directly with patients in their homes while remaining connected to an experienced interdisciplinary team. Your guidance, advocacy, and community knowledge can make a lasting difference for patients and their families.

What You'll Do

• Assess patients' psychosocial, emotional, financial, and environmental needs
• Complete initial social work evaluations and follow-up visits
• Provide counseling, education, advocacy, and emotional support
• Connect patients and caregivers with community programs and financial resources
• Identify barriers affecting patient safety, recovery, or independence
• Help patients and families navigate healthcare and community services
• Participate in developing and updating individualized Plans of Care
• Collaborate with physicians, nurses, therapists, and other care team members
• Support discharge planning and continuity of care
• Communicate concerns or changes affecting patient well-being
• Complete accurate and timely clinical documentation

What You'll Bring

• Master's or Doctoral degree in Social Work from a program accredited by the Council on Social Work Education
• Current Maine Social Worker license in good standing
• At least one year of social work experience in a healthcare setting
• Strong counseling, communication, documentation, and advocacy skills
• Knowledge of community programs and available patient resources
• Ability to work independently in patients' homes
• Willingness to travel throughout the assigned service area with 50% travel required
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations

The base salary for this position is $70.00 to $80.00 per point, based on the company's good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
