Speech-Language Pathologist - Contract

Skills required: Swallowing difficulties, swallow studies, develop and evaluate care plans, identify aspiration risks, determine appropriate diet (soft, mechanical, etc.), patient/family education. Required License, National Certification, Certs: State license, BLS, driver s license, insurance, vehicle, CCCs (certification of clinical competency, received after CFY).

Shift: 8hr Day Shift - 40

Shift Information: 0800 - 1700 (Monday - Friday)

 

Clinical Interviewing- Yes – manager to conduct final interview

On Call, Call Back Requirements: No call

Weekend Rotation: None

Holiday Requirements: No

Floating Requirements: Yes, this is a home health position

About Facility:

• This is a home health-based position
• Will cover parts of Anson and Union counties

Unit Information:

• Beds: N/A
• Case load: cases based off point system. 5-6 points per day

Minimum Years of Experience required: 2 years. Need to have at least 3 months of Home Health experience (will accept internship experience for this)

• Will you accept a first-time traveler: No

Patient Types/Common Diagnoses:

• Adults only, mostly geriatric
• CVA, TBI, dysphagia, apraxia, speech and cognition impairment, cancer patients, aphasia

Required License, National Certification, Certs:

• State license, BLS, driver s license, insurance, vehicle, CCCs (certification of clinical competency, received after CFY)
• Master s degree or above

Support within the Department:

• Another SLP available via phone if needed
• Tuesdays - Care Consult with interdisciplinary team to discuss cases
• Manager/Supervisor
• Scheduler to schedule cases

Technology/Equipment:

• EMR: HomeCare HomeBase (HCHB) – required
• Unit specific equipment: laptop, phone, any tools for visits

Floating: If yes, where: Yes, this is a home health position

Orientation (facility and unit):

• NEO
• 1 week of orientation with SLP teammate

Scheduling:

• Weekend rotation: None
• On call: No
• Holiday Expectations: No
• Approval of RTO: ok to approve
• Shift times: 0830-1700
• Schedule cycle: Monday – Friday
