This SpEd Transition Coach role involves independently planning and implementing community-based programming to promote student independence, employment success, and quality of life for students transitioning from high school to adulthood.
Develops partnerships with parents, community agencies, post-secondary institutions, and businesses to meet students' needs and IEP goals.
Executes IEP goals in community settings, coaches students on job skills, and assists in job placement and life skills development.
Maintains communication with parents, monitors student progress, and evaluates programming effectiveness.
Builds community resources, advocates for students, and educates community members about disabilities and services.
Participates in IEP meetings, special events, and team meetings, while documenting student activities and progress.
Qualifications include experience working with individuals with disabilities, high school diploma or equivalent, and CPR/First Aid certification. Preferred qualifications are an associate’s degree or college credits. The position offers full-time benefits, a schedule of 37.5 hours/week, and requires effective communication, organization, and teamwork skills. The work environment is primarily community-based, involving physical activity and driving, with some office and classroom time.
