Your Role
The Care Management Clinical Social Work team helps Blue Shield members with complex social determinants of health by conducting psychosocial assessments, developing care plans, and providing intensive interventions. The Care Management Social Worker will report to the Manager and/or Senior Manager of Care Management Clinical Social Work. In this role you will be assessing members and linking to appropriate levels of care, linking to community resources, and coordinating with BSC nurses as well as BSC providers.

In this role, you will:
Your Knowledge and Experience
MS or MA in social work, or other related behavioral health field
Current CA LCSW, LMFT, LPCC, or other related clinical licensure
3 years of experience with behavioral health and substance use treatment
Experience conducting thorough assessments, developing care plans, and providing necessary interventions identified
Proficient with Microsoft Excel, Outlook, Word, Power Point, and the ability to learn and utilize multiple systems/databases
Excellent analytical, communication skills, written skills, time management, and organizational skills
