Social Worker - Rehab Focus (BV)

Those services include assessments, care plan development, coordination of treatment plans, individual counseling, assistance to Villagers, families, and representatives with financial and legal issues, coordinating of care conferences, supports, and transitional/relocation planning. Reporting to the Social Services Manager, the Social Worker provides a variety of services designed to meet the Psychosocial and Social Wellness needs of Villagers at Brewster Village.

Reporting to the Social Services Manager, the Social Worker provides a variety of services designed to meet the Psychosocial and Social Wellness needs of Villagers at Brewster Village. Those services include assessments, care plan development, coordination of treatment plans, individual counseling, assistance to Villagers, families, and representatives with financial and legal issues, coordinating of care conferences, supports, and transitional/relocation planning.

This is a full time position working from 8:00am-4:30pm Monday-Friday.  No weekends, holidays, or on-call.  The position will be working in our Short Term Rehab area.  No remote work is available.  All work will be on-site.
