    Job Title: Social Worker

    Employment Type: PRN (Weekday and Weekend availability, required to work 2shifts/month, 1 holiday/yr.)

    Location: St. Mary's Medical Center

    Position Purpose

    The Social Worker provides direct service to patients and families and works with community agencies, physicians and hospital staff in order to provide psychosocial support and to facilitate discharge planning. Modifies approach based on age-specific needs of patient population to include child, adolescent, adult and older adult.

    As a "Social Worker" you will:

    • Demonstrates understanding of patient's current and prior functional status and psychosocial situation through documented assessment. Updates assessment information as new information is obtained.
    • Collaborates with physicians, nursing and the Care Management team regarding discharge planning.
    • Serves as a resource person to the nursing staff, referred patients and their family members, and physicians.
    • Communicates psychosocial information and needs to members of the health care team as appropriate for the continuum of care.
    • Reassess discharge planning needs daily and documents assessment in clinical documentation system. Incorporates cultural, age-appropriate, family and environmental data into assessment and teaching.
    • Ability to perform diversified duties with time limitations with a high degree of accuracy
    • Must be able to work independently and on a team.
    • Demonstrates ability to use problem solving, critical thinking and priority setting skills

    Minimum Qualifications:

    • Bachelor's Degree in Social Work/Services
    • Master's Degree (preferred)
    • Minimum of one (1) year experience in a hospital or health care facility

    Non-benefit eligible Roles

    Under 0.3 FTE (16 hours weekly) or per diem roles
