The ideal candidate will have a strong background in geriatrics and hospice care, with experience in an outpatient setting.This role involves delivering a wide range of social work services to hospice patients and their families, including:Conducting initial emotional, spiritual, and psychosocial assessmentsProviding ongoing counseling and supportOffering bereavement servicesParticipating in community education, outreach, and referralsAs a key member of the Interdisciplinary Group (IDG), the Hospice Social Worker plays an integral role in assessing patient needs, developing individualized care plans, and ensuring compliance with hospice care standards.The position will cover our Ridgeland and Vicksburg, MS office location area, including the surrounding and outlying communities.
