    Support Resident Well-Being and Make a Difference: Social Worker

    Are you a dedicated social work professional ready to champion emotional wellness, coordinate seamless discharge planning, and provide individualized care for our residents? The primary purpose of our Social Worker is to assist in planning, organizing, implementing, and evaluating the Social Services Department in accordance with current federal, state, and local standards, guidelines, and regulations. Directed by the Director of Social Services and/or Administrator, you will ensure that the medically related emotional and social needs of each resident are met and maintained on an individual basis.     About the Role

    As our Social Worker, you will be specifically responsible for all social service functions on your unit, working alongside a social service assistant who provides the majority of LTC social service needs. You will take the lead on discharge planning, MDS completion, Level of Care and Rehab/UR meetings, APS referrals, admission contracts, and interdisciplinary care planning to ensure our residents thrive.


    Key Responsibilities

    • Unit Social Services & Care Planning: Overseeing unit-based social service functions, completing social service sections of the MDS, developing and implementing social service care plans, and conducting admission and ongoing assessments.


    • Discharge Planning & Coordination: Coordinating comprehensive discharge planning for all residents within policy guidelines, managing Rehab/UR meetings and required documentation, and keeping admission contracts and addendums current for skilled and long-term care residents.


    • Assessments, Applications & Committee Work: Serving on the Level of Care committee, assisting residents with MaineCare applications, ensuring referral assessments and Mod Vs are current, and taking the lead role in APS referrals on the unit.


    • Interdisciplinary Collaboration & Family Support: Facilitating family meetings, collaborating with nursing, therapies, dietary, activities, and management to meet resident needs, and maintaining positive, professional relationships with peers and residents.


    • Compliance & Professional Standards: Adhering to professional standards, federal/state/local regulations, productivity guidelines, and attendance policies; maintaining strict resident confidentiality; and engaging in ongoing education to keep your licensure and job knowledge current.

    Qualifications & Requirements

    • Education & Licensure: Bachelor's degree in social work or another social science from an accredited university, and a current Maine Social Work license.


    • Experience & Desires: Long-term care and skilled care experience preferred, accompanied by a genuine experience with and desire to provide services for people, especially those who are elderly and chronically ill.


    • Skills & Abilities: Strong verbal and written communication skills; ability to work in an interdisciplinary environment, problem-solve, take responsibility, and adapt to changing needs with independence, flexibility, and good judgment.
