Social Worker- Permanent(Float)

Services include: adjustments to illness counseling, safety assessments, discharge planning, identification of obstacles and barriers to discharge through proactive screening and assessments, and coordination with the multi-disciplinary team. Responsibilities: Early Identification, high-risk screening, and readmission screening of all patients.

• MD LMSW or higher
• 1+ years of discharge planning experience in an inpatient setting
• Must have EPIC experience
