Job Summary and Responsibilities

As our Social Worker you will proactively assess, develop, and implement comprehensive plans of care in collaboration with patients, families, and an interdisciplinary team. Your expertise will bridge internal and external settings, ensuring a holistic approach to patient well-being and successful outcomes. You will champion patient goals, considering biophysical, psychosocial, environmental, financial, and discharge planning needs.

Every day you will collaborate with care partners to craft appropriate discharge plans and secure timely referrals. You'll serve as a vital advocate for patients and families, procuring essential services and acting as a key liaison to post-hospital care and community health resources. Moreover, you will educate patients, families, and medical staff on available support, providing application assistance for crucial resources.

To be successful in this role, you will possess strong clinical social work skills, a deep understanding of patient advocacy, and proven experience in discharge planning and case management. Your collaborative spirit, excellent communication, and dedication to community resource navigation will be essential in empowering patients and facilitating seamless transitions of care.

Eligible for up to a $3,750 sign-on bonus!

Schedule: Every other weekend; three 10-hour shifts per week (12:00 AM-8:00 PM). Provides primary coverage for the ED and NICU and floats to inpatient units

as required.

Job Requirements

  • Certified Social Worker (CSW) in Nebraska upon hire
  • Minimum of a bachelor's degree in Social Work
  • Basic Life Support (BLS) from the American Heart Association upon hire
