Responsibilities:
Coordinating the care and service of the patients, promoting quality care, effective utilization of resources, and assuming a leadership role with the interdisciplinary team to achieve desired goals and resource outcomes.
Assessing, planning, implementing, and evaluating the delivery of services and working collaboratively with all members of the interdisciplinary team.
Providing therapeutic counseling for patients, families/significant others, as well as members of the interdisciplinary team to enhance coping and functional skills.
Qualifications:
Job Requirements
Master's Degree in Social Work.
Licensed by the State of Texas, or obtain licensure within 6 months of hire.
