Formulate and implement the psychosocial and/or care coordination/discharge planning care plan for identified patients as appropriate to the ages of the patients and to the developmental life cycle stage in conjunction with the medical and interdisciplinary team and the patient/family. Assess the needs of patients referred/identified as appropriate to the ages of the patients and to the developmental life cycle stage (Neonates, Pediatrics, Adolescents, Adults, Geriatrics).
