Seeking a Master's-level Social Worker to support inpatient services.  This full-time role focuses on assisting patients with psychosocial needs, discharge planning, and coordinating care across the healthcare continuum.

Position Highlights:

• Provide crisis intervention and short-term counseling for patients and families
• Facilitate discharge planning in collaboration with interdisciplinary care teams
• Assist patients in accessing medical and community resources
• Support care transitions to help patients achieve optimal health outcomes

Required Qualifications:

• Master’s degree in Social Work or Counseling Psychology
• Minimum 1 year of experience as a Social Worker or Counselor
• Hospital experience (2+ years) preferred

Compensation:

• Salary range: $85,904 – $133,348 annually
