    Job Qualifications (Knowledge, Skills, and Abilities)

    • · Education: Bachelor degree in Social Work
    • · Licensure / Certification: Current LSW license to practice as a social worker in the State of Ohio
    • · Experience: Minimum 1 year experience in discharge planning in an acute care setting or an ambulatory setting.
    • · Effective Communication Skills
    • · Strong organization skills, self-directed, flexible, able to adapt to rapidly changing regulatory requirements.
    • · Possesses excellent clinical assessment and analytical skills.
    • · computer competency
    • · Possesses effective interpersonal skills, can work across departmental boundaries, facilitates problem resolution, and maintain a professional demeanor in difficult situations.
    • · Knowledge of the functional operations of utilization management, discharge planning, care coordination and transitions of care
    • · Understanding of pre-acute and post-aute venues of care and post-acute community resources.
    • · Ability to work independently and exercise sound judgement in interactions with physicians,payers, colleagues and patient and their families.
