The Medical Social Worker (BSW) provides a professional, trauma informed psychosocial assessment addressing psychological, social, emotional, religious, cultural, financial, and health related needs that impact patient care and discharge planning. The Social Worker utilizes strong communication skills to assess and address barriers, coordinate resources, and support safe transitions of care.
