Services include, but are not limited to, adjustment to illness counseling, safety assessments, discharge planning, and identification of obstacles and barriers to discharge. Description : Early identification, high-risk screening, and readmission screening of all patients.

Social Worker- Hospital Setting
Inpatient Units- 13 weeks, M-F, 8:30-5p

Required: 

• MD LMSW or LCSW-C
• Must have recent hospital discharge planning experience in Med/Surg or ED
• EPIC experience.
