  • We are looking for a Social Worker with 2 Professional References.
  • Working Days: Wed, Thurs, Fri, Sat, Sun
  • Social work license.
  • MSW
  • Discharge Planning/Crisis Intervention/Monitor Length of stay for Goals Birth & Women's Unit/ED/Medical/Surgical/ICU Rotating throughout facility.
Roles & Responsibilities:
  • Implement and monitor patient care plans.
  • Monitor, record and communicate patient conditions as appropriate.
  • Serve as a primary coordinator of all disciplines for well-coordinated patient care.
  • Note and carry out physician and nursing orders.
  • Assess and coordinate patient's discharge planning needs with members of the healthcare team.
