JOB OPENING: Social Worker – Manager in Skilled Nursing Facility - Full Time
LOCATION: Northeast Minneapolis, MN
Catholic Eldercare is seeking an organized, person-centered Social Worker to join our team in coordinating services and meeting the psycho-social needs of our residents and their representatives.

Responsibilities:
- Supervises and leads social work team.
- Responsible to complete Admission Paperwork including obtaining face sheet demographic information initially and ongoing.
- Identify the need for medically related social services and assist with the provision of these services.
- The Social Worker identifies the psycho-social and emotional needs of residents and assists with care planning
- Coordinate assigned resident case load and charting.
- Schedule, notify residents/representatives and participate in resident care conferences/special meetings with the multi-disciplinary team and provide necessary psycho-social information specific to each resident. Complete assigned MDS interviews. Complete social services portion of MDS Assessments as required.
- Maintain current knowledge of government regulations and community, public and private services directly related to the financial and psycho-social needs of residents (health insurance Medical Assistance Pending Medical Assistance, PAS, ACG programs, etc.)
- Respond to resident/family/staff concerns using discretion, confidentiality and judgment to involve all necessary persons and coordinate special care conferences when deemed necessary.
- Advocate for residents’ rights.
- Coordinate and update potential discharge plans for each resident.
- Perform all duties in a manner that is consistent with quality standards, infection control, and safety measures, which includes using Standard Precautions, at all times.
- Participate in and or may present job related in-service programs, new employee orientation, mandatory training, and attend meetings in the interest of improving skills, acquiring knowledge, and meeting state and federal regulations.
- Remain current on tuberculosis screening.
- Performs other related duties as assigned.
Qualifications:
Required Qualifications:
- 1-3 years Social Worker licensed by the Minnesota Board of Social Work.
- 1 year previous supervisory experience preferred.
- Ability to supervise newly licensed Social Workers.
- Ability to pass NetStudy 2.0 background screening.
- Experience working with individuals experiencing dementia and cognitive decline (preferred).

Job Context: Salary $70,720.00 to $81,120.00 per year based on experience and qualifications, Exempt (not eligible for overtime)
In-person position, on-site, 100% time.
