    Social Worker - One Care

    Are you interested in making a profound impact on the lives of underserved individuals and families living with complex and/ or chronic medical and behavioral conditions?

    • Are you interested in a loan repayment program and funding for license renewals?
    • Can you commit to work on a Primary Care Team Model as well as with clients independently to provide appropriate follow-up?

    In this role you will:

    • Be responsible for addressing the social service needs of One Care members.
    • Provide follow-up sessions as needed with One Care health center clients or coordinating services by phone.
    • Provide brief counseling, advocacy, & resource referrals as needed.
    • Participate in case conference as needed.
    • Conduct home visits alone or in conjunction with member(s) of the interdisciplinary team as appropriate, either at the patient's home or in other locations in the community.
    • Be an active & integral member of the primary care/BH team.
    • Advocate for patients with outside agencies such as social service organizations, schools, etc.
    • Provide crisis intervention as needed.

    The successful applicant will offer:

    • Human services experience desired.
    • Ability to work with children & adults, & the elderly.
    • Minimum of a bachelor's degree in the human service field (e.g., psychology, sociology) required, BSW preferred; bilingual/bicultural candidates strongly desired.
    • LSW required
    • History of good attendance & positive work attitude.
