Position Overview
The Medical Social Worker (MSW) is a key member of the interdisciplinary hospice team, providing psychosocial assessments, emotional support, counseling, and resource coordination for patients and their families. This role helps address the social, emotional, cultural, and practical challenges that may arise during serious illness and end-of-life care.
Key Responsibilities
Conduct comprehensive psychosocial assessments and ongoing evaluations
Provide counseling and emotional support to patients, families, and caregivers
Assist with advance care planning and community resource referrals
Collaborate with physicians, nurses, chaplains, and other hospice team members
Maintain accurate documentation in the electronic medical record (EMR)
Support patients and families through grief, loss, and end-of-life transitions
Qualifications
Master's Degree in Social Work (MSW) from an accredited program
Hospice, palliative care, home health, or healthcare experience preferred
Strong communication, organizational, and interpersonal skills
Comfortable using EMR systems and computer applications
Spanish-speaking preferred but not required
Valid California driver's license, current auto insurance, and reliable transportation
