Description

Rome Health is seeking a Licensed Social Worker or Licensed Mental Health Counselor to join our hospital-based behavioral health team. This role provides compassionate, patient-centered mental health services to individuals and families across the continuum of care.Position OverviewThe clinician in this role will provide individual, group, and family therapy while completing clinical assessments and supporting treatment planning. This position plays a key role in ensuring patient safety, coordinating care, and connecting patients with appropriate resources and services.Key ResponsibilitiesProvide individual, group, and family counseling sessionsConduct psychotherapy, intake screenings, and comprehensive clinical assessmentsComplete safety planning and provide treatment recommendationsParticipate in treatment planning and ongoing patient supportUtilize evidence-based assessments and therapeutic interventionsProvide case management, referrals, and linkage to community resources as appropriateAssess risk, including suicidal and homicidal ideation, and develop appropriate safety plansSupport patients in identifying strengths, needs, and treatment goalsCollaborate with families and care teams as clinically indicatedQualifications & KnowledgeKnowledge of mental health disorders and evidence-based treatment approachesAbility to complete risk assessments and develop effective safety plansFamiliarity with DSM-5 criteria and substance use disorder (SUD) diagnosis supportUnderstanding of professional ethics and standards of careKnowledge of OASAS 822 regulationsStrong case management skills, including knowledge of community resources and referral processesAbility to assess family dynamics and incorporate them into treatment planningRequired CredentialsCurrent New York State licensure as LMSW, LCSW, or LMHC (required)Rome Health is committed to providing high-quality, compassionate behavioral health services to our community and supporting clinicians in delivering meaningful, patient-centered care.
