Intake Social WorkerThe Intake Social Worker at the Jona Goldich Multipurpose Senior Center provides responsive and client-centered support to individuals seeking social services, community-based assistance, and information and referrals. This role is responsible for managing both phone and walk-in inquiries, conducting intake assessments, and providing short-term case management support as needed. The position also supports outreach activities, including community workshops, information sessions, and engagement events designed to connect older adults and adults with disabilities to essential services and resources.Qualifications:Bachelor's degree in Social Work required or related human services field.Prior experience in client engagement, intake services, customer service, or social service delivery requiredStrong research skills to identify and connect clients with appropriate community resourcesExcellent verbal and written communication skillsBilingual in English/Russian preferred or willingness to utilize language line interpretation servicesDemonstrated ability to work with diverse, multicultural populations in a culturally sensitive mannerStrong interpersonal skills and ability to provide non-judgmental client supportComfortable presenting in group settings and supporting outreach events and workshopsAbility to work collaboratively within a multidisciplinary teamStrong computer proficiency, including MS Office and ability to learn electronic health record (EHR) systemsAbility to maintain professional boundaries with clients and staffValid California Driver's License and proof of current auto insurance requiredMust successfully pass criminal background check, including Live Scan fingerprinting, prior to employmentResponsibilities/Essential Functions:Provide intake, engagement, and time-sensitive support to incoming phone calls and walk-in clients at a multipurpose senior center serving older adults and adults with disabilitiesConduct intake assessments, including bio-psycho-social and environmental evaluationsProvide information, referrals, and service linkage based on client needsTriage clients for social work case management and coordinate appropriate follow-up servicesAssist clients with applications for housing, public benefits, and other supportive servicesConduct home visits as required to assess client safety and complete intake documentationSupport outreach efforts, including workshops, presentations, and community eventsCollaborate with case management, nutrition, and activities staff to enhance client engagementResearch and identify community resources to support case management staff and client needsAssist with completion of grant applications and benefit-related documentationMaintain timely and accurate documentation in the electronic health record (EHR) system, ensuring all notes are completed dailyParticipate in multidisciplinary team meetings and weekly supervision sessionsManage multiple priorities effectively while maintaining quality service deliveryPerform other related duties as assignedJewish Family Service is proud to provide equal employment opportunities to all employees and qualified applicants without regard to race, color, religion, sex, sexual orientation, national or ethnic origin, age, disability or status as a veteran.This position reports to Jona Goldrich and Felicia Mahood Program Director
