About the Position

The Social Worker II/III will serve as a Lead Care Manager in the County of San Luis Obispo Public Health Departments Enhanced Care Management (ECM) Program. This position provides person-centered care coordination for Medi-Cal members with complex medical, behavioral health, and social needs, including individuals experiencing homelessness, justice involvement, and other barriers to care. This position combines office-based and field-based work throughout San Luis Obispo County. Lead Care Managers regularly meet with individuals in homes, shelters, hospitals, clinics, correctional facilities, and other community settings. This is a unique opportunity to make a meaningful difference in the lives of individuals with the highest levels of complex social and health needs.

Responsibilities include conducting comprehensive assessments, developing care plans, coordinating services across health care and community systems, advocating for clients, documenting services in the electronic health record, and collaborating with multidisciplinary partners to improve health outcomes.

The Ideal Candidate

The ideal candidate is a compassionate and collaborative social worker with strong assessment, care coordination, and advocacy skills. They should be comfortable working with individuals experiencing homelessness and/or complex medical, behavioral health, housing, and social challenges and be able to build effective relationships with clients and community partners.

Experience with Medi-Cal populations, care coordination, public health, housing resources, behavioral health, or justice-involved populations is highly desirable.

The successful candidate will be organized, adaptable, and committed to providing high-quality, person-centered services as part of an interdisciplinary team. They should be comfortable working in a dynamic environment where priorities, policies, and workflows may evolve over time. The ideal candidate is open to feedback, embraces continuous learning, and is able to adjust to change while maintaining professionalism, accountability, and a positive, solution-focused approach.

Strong communication skills, sound judgment, and the ability to balance competing priorities while maintaining accurate documentation and timely follow-through are essential for success in this role.

Examples of Job-related experience: Experience in a public or private agency that includes supporting or managing a caseload comprised of people with various types of social service problems, by properly applying the laws, rules, and regulations governing the operation of social service agencies.

Pension Benefit and Employee Contribution Rates:

The Countys pension is a defined benefit plan that provides a guaranteed monthly pension payment for life based on your time in service, wages, and other factors. The County is the primary contributor to the pension fund, but the health of the pension fund also depends on mandatory employee contributions. Employees are required to pay a percentage of their base pay towards the pension fund, ranging from 5% to 19%. This amount is deducted from your paycheck pre-tax every pay period. Healthcare Benefits and Cafeteria Contributions:

The County offers comprehensive medical coverage to you and your family, along with two dental plan options and vision coverage. The amount of the cafeteria contribution varies by Bargaining Unit (BU) and the extent to which employees enroll their dependents. If the cafeteria amount is greater than your selected premiums, the remainder of the cafeteria is paid out to you in your paycheck as taxable income.
