    Harbors Home Health & Hospice is seeking a compassionate, professional Social Worker to join our team serving patients and families throughout Grays Harbor and Pacific Counties.

    This position provides essential psychosocial support to patients and families receiving home health and hospice services. The Social Worker works collaboratively with nurses, therapists, aides, providers, chaplains, volunteers, and other members of the interdisciplinary team to help patients remain safely and comfortably in their homes while addressing the emotional, social, financial, and practical challenges associated with illness, aging, disability, and end-of-life care.

    Required qualifications:

    Masters degree in Social Work or

    Bachelors degree in Social Work, psychology, sociology & one year experience in a healthcare setting

    Drivers license with acceptable drivers abstract and current vehicle insurance

    Ability to travel throughout the assigned service area

    Ability to work independently and effectively manage a community-based caseload

    Strong written, verbal, organizational, and interpersonal communication skills
