AuthoraCare Collective is currently seeking a Social Worker Home Care supporting our Burlington service area. This is a full-time position; Monday - Friday 8:00am - 5:00pm.

Job Summary:

Responsible for providing comprehensive assessment of the social, emotional and practical needs of the Hospice patients and their support systems. Provides advanced counseling, education and referrals to address patient/family issues of grief and loss, adjustment, care needs and end of life planning. Provides educational programs to the community. Functions as a member of an interdisciplinary team

of care.

Education/Qualifications:

• Master's degree in social work
• LCSW OR LCSWA (or in the process of obtaining) required
• One year of experience in social work experience in a mental health or medical setting is required.
• Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.

Key Responsibilities:

Delivery of Care

• Prepares psychosocial assessments.
• Develops and carries out individualized plans of care related to the grief and loss, adjustment, care, and end of life planning needs of patients, families, and caregivers.
• Identifies appropriate resources to meet patient/caregiver/family needs.
• Serves as an advocate for patients and families.

Interdisciplinary Group Process

• Actively participates with the interdisciplinary group (IDG).
• Works effectively with other IDG members with a focus on collaboration in problem-solving patient needs and working with the patients' plan of care.
• Provides effective report to the IDG based on regulatory requirements for patient care planning evaluation.
• Maintains effective relationships and partnerships with community healthcare providers, agencies, and faith communities.

Documentation

• Completes required documentation according to licensure and certification requirements and agency standards including accreditation standards.
• Ensures that notes and patient records are complete, accurate, current, and clear.
