JOB SUMMARY:

Responsible for providing Social Work services and Discharge Planning to patients and their families of Northern Maine Medical Center, in addition to some case management of chronic conditions in the primary care setting.

ESSENTIAL FUNCTIONS:

  • Responsible for performing psychosocial assessments on patients to determine social service and discharge planning needs and completes all associate paperwork timely and accurately.
  • Collaborates with physicians, nurses, family members (if appropriate), patients and/or the Interdisciplinary team to assist in the discharge planning process and to formulate the discharge plans and/or the patient's treatment plans including attendance at patient related conferences/meetings as needed or required.
  • Demonstrates knowledge of Social Work, Discharge Planning, and resources available in the community. Assist in making appropriate referrals to support services, educating patient and family about need for and availability of service and obtaining a release of information from the patient and or guardian appropriate to making referrals.
  • Assists with/provides coverage for utilization review and case management functions as needed.
  • Develops and maintains professional working relationships with other hospital departments, outside agencies, and other Hospital/facilities. Consults with other disciplines as needed.
  • Demonstrates an ability to be flexible, organized and function under stressful situations.
  • Participates in relevant continuing education including mandatory in-service programs.
  • Treats patients and families with respect and dignity, maintains strict patient confidentiality.
  • Participates in the department Performance Improvement programs.
  • Documentation meets current standards and policies.
  • Maintains knowledge and skill to provide care appropriate to the age of the patient's served.
  • Maintain flexibility of work hours to meet the needs of the patients and the organization.
  • Maintains current Maine Social Worker's license.
  • Complies with and enforces all NMMC policies and procedures.
  • Performs other duties as assigned.

EDUCATION, TRAINING AND EXPERIENCE:

Minimum bachelor's degree in Social Work or other related field. Must be a Licensed Maine Social Worker.
